Services to community that are consistent with integrated service delivery principles
Planning and delivering Group Services Programs appropriate to client’s needs
Acting as a Keyworker to assist our clients to set and achieve realistic and individual goals
Strong relationships with clients, their carers and/or family and other support networks, holding a collaborative approach to your work
Reporting and recording all program information as required
A people-centred approach that enhances and maintains the abilities and independence of individuals
Innovative and creative solutions to accommodate changing circumstances
- Requirements:
Strong communication skills for building trust and rapport with clients
Demonstrated skills and knowledge working in Community Care that aims to enhance an individual’s independence
Experience working with older people with memory loss and disability and knowledge of issues relating to disability and ageing
Passion and commitment to your role and driven to achieving great client outcomes
Good written and verbal skills along with a basic knowledge of IT use
Certificate III in Home and Community Care, Aged Care, Disability Support or equivalent
Current driver’s license
