- Job Description:
Accepting referrals from providers, individuals, carers seeking clinical evidence to complete the access process, undertaking a pre-screen to determine likely eligibility for the NDIS and offering support to complete the access process, as required.
Collecting and collating all necessary evidence and completing the Access Request Form
Assisting with submitting Access Request Form to the NDIA and acting as a contact person for NDIA queries
Waiting list management intervention which includes: call back, cross sector referrals, gathering additional information etc.
Perform risk assessments (including referral to crisis intervention services, where necessary).
Maintain accurate individual consumer files and data bases (Carelink)
Facilitation of small group wellbeing programs (as required)

Drivers licence
Police check (refer to PD) – disclosable outcomes considered
A Disability Worker Exclusion Scheme Check will be undertaken prior to commencement
Neami National will require, as part of our condition of employment, that all candidates applying for any position must be fully vaccinated for COVID-19 (or have received one dose of an approved COVID-19 vaccine and have a scheduled appointment to receive their second dose.)
